Our Health Information Management Department can release records from Gulfport Hospital, Ocean Springs Hospital, Pascagoula Hospital, and any Singing River Health System-owned clinic.
Health Information Management
Records can be requested and picked up from Ocean Springs Hospital or Pascagoula Hospital for any hospital or Singing River owned Medical Clinic. For Gulfport Hospital records, you must contact the Gulfport Office.

Requesting Medical Records
We release records for Singing River Health System, which includes: Gulfport Hospital, Ocean Springs Hospital, Pascagoula Hospital, and any Singing River Health System owned clinics. To have a copy of your medical records sent to another facility or to obtain a copy for other purposes, please complete the following steps.
- Download the Authorization Form
To have a copy of the Authorization Form mailed to you, please call (228) 809-5197. - Complete and sign the Authorization Form and provide a clear copy of your driver’s license or ID.
- Deliver, fax or mail the signed Authorization Form and proper identification to:

It is the policy of the Release of Information Office to complete requests in the order in which they are received. All requests are processed in a timely manner; however, certain circumstances may cause delays in processing and can take up to 30 days to process.
The Medical Records Department will contact you by phone or mail when your records are ready and payment is due.
Medical Records Service Fees
There is no charge for having your medical records sent directly to another healthcare facility/provider. However, there are minimal fees (per page) for obtaining medical records for other purposes. See below for the itemization of charges.

Certified Copies of Birth Certificates
To request certified copies of Birth Certificates or for changes/corrections, you may use the following options:
- Call Mississippi Vital Statistics at (601) 206-8200 and follow the prompts
- Order online or download the application from www.msdh.ms.gov
